If you are new to the NDIS you will probably have received a crash course in how NDIS funding packages work. There are 2 principles to be aware of. You will have:
With your individualised funding package, you can choose to enter into a Service Agreement with one or all of the collaboration partners OR select an alternative service provider(s). Your choice. You can also make changes to your team at any time if the fit isn't right for you.
The NDIS will fund accommodation in a few circumstances. These are :
Your NDIS funding plan will provide an allowance to meet your needs that relate to your disability.
If you are new to the NDIS, this process will begin in hospital with support from your social worker, NDIS liaison and possibly a support coordinator.
If you already have your funding plan, get in touch to see if you are funded appropriately for us to assist you right away, or help you with changes to ensure you can benefit from this transition service.
SN Services and Anderson Neurological and Development Services will assess each applicant for suitability for the service and together develop a schedule of supports for your stay with us.
Once your stay is complete, both service providers may continue to support you in the community if you valued the service you received and wish to keep services going.
As you will have your own fully equipped apartment, you will fund your own meals, cooking, cleaning, and laundry services.
For some people, you may have a funding allowance for assistance with any or all of these things. It will be unique to your circumstances.
So, if it is Taco Tuesday, bbq on the sundeck, Uber Eats, or a counter meal at the Carine Tavern, meal times will be what you want them to be, not just what is served up!
One additional benefit is we do have laundry facilities on-site, so if you have the SN Services team assisting, the team may wash your linens and clothes in our laundry to avoid the noise and intrusion in your home.
You will also contribute to your accommodation and utilities costs. The contribution is currently $250 per week plus utilities or $350 per week with utilities included.
If you have found us, and do not yet have a funding plan in place we highly recommend you reach out to our team to consult about services and establish your needs to help build a plan with your hospital team that allows you to transition from hospital to our Hospital2Home service as a stepping stone to your permanent home.
The uncomfortable reality is the NDIS needs funding efficiencies. By choosing this transitional service you get the benefit of your own brand new apartment to live in rather than a shared living arrangement, while you still share supports with others.
In fact, your time at our service will likely be more affordable for the NDIS while you are with us than when you return home and can't share supports. We are here to make sure your stay is affordable. The gains you make during your stay will hopefully deliver savings in your daily support needs long term as you launch better equipped for the daily challenges ahead.
If you are privately funded, being supported by another insurance scheme, or funded by the State government the team can assist. If you fall into this category, we will work with you to bundle a package that meets your needs and budget.
Our collaboration partners are registered with the Insurance Commission of WA.
